  • Hazard Tree Evaluation Using Survey123 (PDF) is an all-new guide for the e-form for inspections of trees in developed forest sites; now with report templates. The e-form can be run on any smartphone, tablet, or computer using the program Survey123 by ESRI. This electronic form can be used as a replacement for of the Rocky Mountain Region's older Hazard Tree Evaluation paper form. The paper form for inspection of trees in developed forest sites was reorganized and reformatted in 2021 for greater efficiency and consistency with the new e-forms.

  • Tree Failures Evaluation Using Survey123 (PDF) is an all-new guide for documenting tree failures in developed sites on National Forests and other locations; now with report templates. The e-form runs on any smartphone, tablet, or computer using the program Survey123 by ESRI. The electronic form is based on the International Tree Failure Database form, but includes only fields common in the Rocky Mountain Region's developed recreation forest sites. There is a paper Tree Failure Evaluation (PDF) form to supplements the new e-form.

  • Each R2 national forest has an ArcGIS Online Hazard Tree group. Please contact your group owner to be added to the group. Users must belong to a group in order to use the Hazard Tree Evaluation e-form and the Tree Failure e-form.
