COVID-19 screening testing information

The Forest Service will follow the Safer Federal Workforce Task Force guidance and the USDA’s COVID-19 Workplace Safety Plan to ensure public health best practices as determined by the Centers for Disease Control and Prevention are in place. As we prepare for more employees to reenter the physical workplace, one such safety measure the Forest Service will soon be implementing is a COVID-19 screening testing program for employees who are not fully vaccinated employees.*

The Forest Service uses the proof of vaccination survey tool as the official database of record for identifying fully vaccinated employees. If you have changed supervisors since you uploaded your information, you must go back into the survey tool and update your info with your correct supervisor. If an employee does not yet have access to a government computer, a supervisor may upload their employee’s information on their behalf here. If you are having difficulty with the proof of vaccination survey tool, please first check with your supervisor and read these FAQs. If you are still having trouble, please contact OCIO.Survey@usda.gov.

Supervisors, it is your responsibility to review your Proof of Vaccination Report. This report lists your employees who have provided proof of their vaccination. By default, if an employee’s name is not found in this report, they will be automatically enrolled in the screening testing program.  Please ensure that your employees have correctly selected you as their supervisor. If an employee has changed supervisors, they must go back into the survey and update their supervisor.

For all employees: If you have not yet uploaded your vaccination data into the survey tool, please do so now. If your name is not on your supervisor’s vaccination report, you and your supervisor will receive a notice of your enrollment into the screening testing program, along with implementation instructions.  

*Employees will be considered fully vaccinated for COVID-19 two weeks after they have received the requisite number of doses of a COVID-19 vaccine approved or authorized for emergency use by the U.S. Food and Drug Administration or that has been listed for emergency use by the World Health Organization. For Pfizer-BioNTech, Moderna, or AstraZeneca/Oxford, that is two weeks after an employee has received the second dose in a two-dose series. For Johnson and Johnson (J&J)/Janssen, that is two weeks after an employee has received a single dose.

A booster shot is encouraged but not required for an employee to be considered fully vaccinated.
