6404 - RESPONSIBILITY

Janice Wayland, Contract Specialist, is the designated Property Management Officer (PMO) on the Hoosier National Forest.

6414.1 - Accountable Property Officer . Accountable Property Officers (APO) for units on the Forest are designated as follows:

Duties and responsibilities of this designation are listed in FSM 6414.1, Item 1 through 8.

6414.31 - Property Record Forms.

Form AD-107 - Report of Transfer or Other Disposition or Construction of Property. The losing district or office will retain the original signed copy. The gaining office will get one copy and the Supervisor's Office will get one copy.

Form SF-120 - Report of Excess Personal Property. The original will be sent to the Supervisor's Office whenever requesting or declaring excess property.

Form AD-112 - Report of Unserviceable, Lost, Stolen, Damaged, or Destroyed Property. The original plus two copies will be sent to the Supervisor's Office. DO NOT COMPLETE BLOCKS 4, 5, or 6. The district will take action after approval by the PMO, then complete Block 5, and return one copy to the Supervisor's Office.

Form OF-16 - Sales Slip. Use this form when property sales are made. APO will prepare the form for the purchaser's signature. The original will be sent to the PMO in the Supervisor's Office.

6414.6 - Physical Inventories . Annual inventories (100% inventory every other fiscal year; 10% during the alternate years) of all accountable property will be taken by all Districts and the Supervisor's Office. These inventories will be reconciled with control records. Inventory printouts will be furnished by the Supervisor's Office in January.

Perpetual inventories are required for tree marking paint with tracer element. FS Form 6500-95 will be used for this purpose.

6446.2 - Inventories and Reconciliations.

1. Inventories

a. Periodic Inventories. Periodic inventories of all buildings and structures will be taken every two years, and identified by the asset number. Send one AD-113 (Physical Inventory Report) with each copy of the inventory signed by the APO or District Ranger.

Buildings are to be numbered by using aluminum numerals, routed signs, or by stenciling.

(1) Size - Size of the numerals are to be 2 1/2" to 3" high.

(2) Color - Colors are to contrast with the building's paint.

(3) Numeral Placement on Building - Numerals are to be placed in a conspicuous location, preferably above the main entrance. For buildings with street numbers, placement may best be above a rear door or on the end or corner of the building.
