HRM: Collect Linc Pass cards when an employee leaves the Forest Service
All supervisors and contracting officer representatives are required to collect an employee’s Linc Pass card when they leave employment with the Forest Service. The AD-139 Final Salary Payment Report form (internal link) can be used to document collection of a Linc Pass card. Please mail the collected Linc Pass card to the HRM Linc Pass Security Officer using the address below.
If the supervisor or representative is unable to collect a Linc Pass card at the time of the employee’s departure, please send an email to the personnel security staff with the employee’s full name and departure date. The personnel security staff will immediately submit the system termination of the Linc Pass card credential based upon the email. When the Linc Pass card is collected, please mail it to the HRM Linc Pass security officer using the address below.
An employee will be allowed to keep their Linc Pass card if they are transferring to another USDA agency.

For proper destruction, please mail collected Linc Pass cards to the HRM personnel security officer using the following address:
