Middle Mountain Trail #608 is a 20-mile linear trail that starts directly across HWY 39 from Rimel Picnic Area and travels south to Neola, WV. This trail climbs Middle Mountain as a narrow footpath. The southern terminus of the trail reaches the North Fork of Anthony Creek, across from Forest Road 96. The route is marked with blue trail blazes. The closest source for drinking water is Pocahontas Campground.

There are 42.6 miles of the ALT traversing the Marlinton Ranger District divided into two segments. One section is from ‘The Dock’ on County Road 23 (Douthat’s Creek Rd) just south of Minnehaha Springs to Huntersville (20.4 miles). The other section is from Huntersville to Sitlington, near Dunmore (22.2 miles). Yellow trail blazes mark its route.
