3,000 Pound Helicopter Swivel Hook Retrofit Information

Please include a completed copy of this Retrofit Information Form with each shipment. This Adobe Acrobat PDF file has been created as an editable text form that can be filled out using the free Adobe Acrobat Reader. To utilize this form, you can either:

• Fill out the form on your computer and e-mail it to us; OR
• Print the form, fill it out by hand, and fax it or mail it to us.

To use Adobe Acrobat Reader to fill out the form, follow these steps:

1. Save the PDF file to your computer by right-clicking the link to the Retrofit Information Form, then choosing Save Target As (if you are using the Internet Explorer browser) or Save Link As (if you are using Netscape or a Mozilla browser). In the Save As dialog box, be sure to remember which folder you are choosing to save the file in so that you can find it on your own computer after it is downloaded. The default folder on most Windows computers is My Documents.
   
   
2. Locate the file on your own computer, and check to see if the file is editable by right-clicking the filename retrofrm.pdf, left-clicking the word Properties, and making sure there is no checkmark in the "read-only" Attributes checkbox at the bottom of the Properties window.
   
   
3. Double-click the filename. This should automatically open the Adobe Acrobat Reader program, if it is already installed on your computer. If the form does not open, click the Get Acrobat Reader icon to download and install a free copy of the Adobe Acrobat Reader. You should have version 5.0 or later in order to use this form fill-in option.
   
   
4. Left-click on the line to the right side of the Agency Name field, and begin typing. To move from line to line you can either point and click with the mouse to choose the appropriate line, or use the Tab key to move forward through the form fields.
   
   
5. Be sure to fill in appropriate names on the Signature lines.
   
   
6. When you have completed the form, you may want to use the File, Save As option to save this completed form under a different name in order to keep your original copy clean for later use.
   
   
7. E-mail the completed form to dhaston@fs.fed.us

If you prefer to print the form and fill it out by hand, you can mail it or fax it:
