B. Description of the Facility to be Conveyed

Write a one-paragraph narrative on the facility and associated site. Complete the below table for each building involved in the conveyance. If this is a conveyance nomination for construction, briefly describe the construction that will take place.

C. Selection Criteria

Describe how the project meets each of the following criteria

1. Readiness—Describe whether facility Master Plan is complete, the NEPA decision supporting the action is complete, if regional funds are available for the cost of the proposal, Civil Rights Impact Analysis complete (CRIA).
2. Contacts with and Support of Elected Officials—Describe discussions with local, state, and federal elected officials and their level of support for the project.
3. Funds—The amount of funds projected to be generated from the sale (best estimate of amount).
4. Impacts to Local Communities—Describe interests or concerns expressed by local, state, or federal agencies or local community groups.
5. Heritage—Verify that the historic evaluation is complete with determination of ineligibility or approved mitigation with SHPO clearance and that disposal is in compliance with Section 106 of the Historic Preservation Act.
6. Benefits to the Agency—Report the reduction in deferred maintenance resulting from the conveyance, describe any hazards or liabilities that are eliminated, and describe partnership opportunities created.

D. Photographs

Include two photographs of each building involved in the conveyance. Each photo must be labeled for identification.
