Accident Investigation Guide: 2005 Edition

1. The top position is occupied by the Designated Agency Safety Official (DASHO) or the official authorizing the investigation.
   
   
2. On the second level is the Team Leader.
   
   
3. On the third level, occupying equal positions of authority, are the Chief Investigator or Qualified Technical Investigator, and the Safety Manager.
   
   
4. On the fourth level at equal positions are the:
   • Documentation Specialist
   • Union Representative
   • Technical Specialists (as needed)
   • Law Enforcement Representative.
