The Accident Investigation Team is responsible for the tasks associated with the first phase of the accident investigation process. The team conducts the following tasks and presents the results to the Accident Review Board:

Cell 1 (top left). Tasks include preparing the:

• Preliminary 24-hour briefing
• Expanded 72-hour briefing
• Safety alerts

Cell 2 (top right). Tasks include establishing the:

• Accident sequence
• Physical and photographic evidence
• Interviews
• Human factors
• Equipment factors
• Environmental factors
• Witness statements
• Findings
• Causal factors
• Contributing factors

Cell 3 (level 2, center). Tasks include writing the:

• Draft Factual Section
• Draft Management Evaluation Section
• Recommendations

Cell 4 (level 3, center). The Accident Review Board (ARB) is then responsible for the tasks associated with the next phase of the accident investigation process. The ARB:

• Accepts or rejects the Factual Section
• Accept or rejects the Management Evaluation Section
• Drafts the Accident Prevention Plan

The ARB then forwards the final reports and a transmittal letter to the Forest Service Chief in the Washington Office.
