Recordkeeping forms:

  • Use OSHA forms 300, Log of Work-Related Injuries and Illnesses, 300A, Summary of Work-Related Injuries and Illnesses; and 301, Injury and Illness Incident Report.

  • Forms are maintained on a calendar year basis.

  • Form 300A, Summary of Work-Related Injuries and Illnesses, for the previous year, must be posted from February through April of each year.
